Refund and Cancellation Policy

Refund Policy

Because many services involve laboratory processing, chain-of-custody procedures, or third-party fees, all sales are final once:

• The process has started, collected, supplies opened, submitted or items shipped

This policy applies to all testing and collection services provided by Square One Lab Testing Solutions LLC.

Cancellation & Rescheduling Policy

Appointments cancelled at least 24 hours before the scheduled appointment may qualify for a partial refund, minus a $25 administrative processing fee.

No-show appointments are non-refundable.

Chargebacks & Payment Disputes

Clients agree to contact Square One Lab Testing Solutions LLC before initiating a payment dispute or chargeback.

Improper chargebacks may result in:

• Suspend current or future services

• Assess reasonable administrative costs associated with this dispute

• Refer unpaid balances to a collection agency

• Pursing legal remedies were permitted by law (bank fees, court fees etc.).

Clients are encouraged to contact us directly regarding any billing concerns so we may work toward
